To make a booking, please email our office with the following details. Clicking the button below will open a pre-filled email in your mail client.

## What we need

- **Procedure date and time** (and estimated duration)
- **Hospital and theatre**
- **Surgeon / proceduralist**
- **Operation / procedure**
- **Patient details:** name, date of birth, any clinically relevant comorbidities
- **A daytime contact number** for any clarifications
